Answer: Personal information in relation to the patient can be signed in a release form. Such as in the case of a court hearing or civil case. Another example would be if the patient is deceased; or for insurance purposes they can use your health information in order to address your current and potential future problems. If it pertains to your current health-related needs, a hospital may call the pharmacy to get a list of your current medications and dosages if you are unable to list them all. That information can than be used with out having any relation to their health care/provider.
